Referring physicians and nurse practitioners are encouraged to have patients explore resources available in the community to support weight loss  prior to a surgery recommendation. This includes lifestyle change, such as those offered through the Primary Care Networks or the Alberta Healthy Living Program.  

When referring to the Calgary Adult Bariatric Surgery Clinic, ensure all relevant supporting documents are submitted with the referral.  
Referral criteria is available on the Alberta Referral Directory.


Eligibility Criteria

Surgery is only available to patients 18-64 years of age.

Adults, age 18 and over with:
  • BMI ≥ 40 kg/m2  with or without co-morbidities
  • or BMI ≥ 35 kg/m2, with at least one obesity-related co-morbidity
AND
  • No severe, untreated  personality disorder, active psychosis or major cognitive impairment.
  • No active substance dependence including nicotine and tobacco products by at least twelve months.

Referral

A complete referral and supporting documentation is required.
Fax the completed referral form and screening tools to Endocrinology and Metabolism Central Access and Triage (403-476-9626).

Communication

After a referral is received and reviewed, the referring provider is informed of the triage decision.
The patient is contacted with information about the clinic orientation and required classes and individual appointments are booked after.
Our clinic staff will communicate regularly with the referring provider.
